Accredited YAKAP pharmacy or GAMOT pharmacy?
People search for a "YAKAP accredited pharmacy", but pharmacies are accredited under GAMOT, YAKAP's medicine benefit. YAKAP accreditation is for clinics. So the pharmacy you need is a GAMOT-accredited pharmacy, sometimes called a GAMOT botika or GAMOT facility.

- Check PhilHealth's list of accredited facilities on the PhilHealth website or the eGovPH app.
- Ask the pharmacy to validate your UPSC. Only accredited pharmacies can do it in the GAMOT App.
What you need to claim
- Your UPSC or the printed GAMOT prescription from your YAKAP doctor.
- A valid government ID.
- If someone claims for you: an authorization letter, the UPSC, and IDs for both of you.

The official PhilHealth source
PhilHealth publishes the program at philhealth.gov.ph/yakap. It lists four YAKAP benefit categories: Primary Care Checkup, Laboratory, Gamot List and Cancer Screening Test, and treats YAKAP clinics and GAMOT providers as separate accreditations.

Yes. Your UPSC can be claimed at any GAMOT-accredited pharmacy within its 90-day validity.
