TrueMed GAMOT App Privacy Policy

What the TrueMed GAMOT mobile app does — and deliberately does not do — with your information.

This policy covers the TrueMed GAMOT mobile app — including the health caravan sign-up and the GAMOT availment form inside it — and the refill request form at refill.truemed.ph. The truemed.ph online store has its own store privacy policy.

What the app itself collects

The TrueMed GAMOT app has no accounts, no sign-in, no analytics, no advertising, and no tracking of any kind. Browsing the medicine list and the benefit guide sends nothing about you anywhere.

There are exactly two places in the app that ask for personal information, both described below: the health caravan sign-up on the UPSC tab and the GAMOT availment form on the Collect tab. Each sends what you enter only after you tick its consent box, only to TrueMed's own systems, and only for that purpose. If you never use either, no personal information leaves your phone.

  • Your language choice and eligibility quiz answers are stored only on your device (local storage) and never leave it.
  • After a caravan sign-up or an availment submission, only its reference code (and, for the caravan, your ticket) is kept on your device so you can see it again — no photo, signature, or answer is stored on the phone.
  • The medicine catalogue is bundled inside the app and works offline.
  • Product images are loaded from our store's content delivery network (cdn.shopify.com). No identifying information is sent with these requests beyond what any image download requires.
  • The availment form asks for camera and photo library permission so you can photograph your prescription and ID. The app never saves anything to your photo library.

The health caravan sign-up

One of the two places in the app that ask for personal information is the health caravan sign-up on the UPSC tab. A health caravan is a consultation day TrueMed brings to a company's own workplace, and the sign-up reserves you an hour of that day.

When you sign up, the following is sent to TrueMed's own server: last name, first name, middle name (if you give one), birthday, sex, department, mobile number, and PhilHealth number. It is sent only after you tick the consent box shown on the form, under the Data Privacy Act of 2012 (R.A. 10173).

  • It is used for that drive only: to give you a batch number and a time, and so our team can identify you when you arrive.
  • It is shared with the HR team or employer that organised the caravan, because they run the attendance list at their own site.
  • It is never sold, never traded, and never used for advertising or tracking of any kind.
  • No account is created. No password. No email address is requested.
  • Your PhilHealth number is shown masked on the ticket (for example ••••••••5678). The app never displays it in full.
  • A copy of your ticket is stored on your phone alone, so it still works with no signal. Deleting the app removes it.
  • If you never sign up for a caravan, nothing leaves your phone from this part of the app.

The GAMOT availment form

In version 1.3 the GAMOT availment form on the Collect tab is part of the app itself; it used to open in a browser. It is how you ask us to prepare your free medicines once you already hold a UPSC code.

When you submit the form, the following is sent to TrueMed's own system:

  • a photograph of your prescription, and the clinic name and UPSC code written on it;
  • a photograph of a valid ID, and which type of ID it is;
  • your last, first and middle name, your birthday, and your email if you give one;
  • your signature, which you draw on the screen;
  • your contact number and delivery address (street, city, ZIP, region, landmark and any delivery instructions);
  • whether you are a Walk In or came through a company or group.

It is sent only after you tick the consent box shown on the form, under the Data Privacy Act of 2012 (R.A. 10173).

  • It is used to prepare your availment: so a pharmacist can check the prescription, so your PhilHealth benefit paperwork can be filed, and so the medicines can be delivered or made ready for pickup.
  • It is stored in TrueMed's own Google Drive and records. It is never sold, never traded, and never used for advertising or tracking of any kind.
  • No account is created. No password.
  • When you take the photograph of the prescription it is sent to our server once, so the clinic name and the UPSC code printed on it can be read and filled in for you. Every field stays editable.
  • Nothing from this form is kept on your phone except the reference code (for example TM-20260907-N87AB) and the date, so you can see when you last submitted. No photograph, no signature and no answer is left on the device.
  • You do not have to allow the camera. A small link under the button opens the same form in a browser instead.

The refill request form

If you choose to open the refill request form (refill.truemed.ph) from the app, it opens in your device's own browser view, separate from the app. To process a refill request, that form asks for your name, birthday, contact details, and signature.

  • Why we collect it: to verify your identity, coordinate the re-issuance of your prescription with your clinic, and dispense and deliver your medicines under the PhilHealth GAMOT/YAKAP program.
  • How it is used: only for prescription validation, dispensing, and delivery. It is shared only as needed to fulfil your request — with your clinic and with PhilHealth's validation systems. We do not sell it or use it for marketing.
  • Retention and your rights: records are kept as required by pharmacy regulations. Under the Data Privacy Act of 2012 (RA 10173), you may request access, correction, or deletion of your personal data by emailing gamot@truemed.ph or calling 0968 188 7569.

Ang health caravan sign-up

Isa sa dalawang bahagi ng app na humihingi ng personal na impormasyon ang sign-up sa health caravan sa UPSC tab. Ang health caravan ay ang araw ng konsultasyon na dinadala ng TrueMed sa mismong opisina ng isang kumpanya, at ang sign-up ang naglalaan sa inyo ng sariling oras sa araw na iyon.

Kapag kayo ay nag-sign up, ipinapadala sa sariling server ng TrueMed ang mga sumusunod: apelyido, pangalan, gitnang pangalan (kung ibibigay ninyo), kaarawan, kasarian, departamento, mobile number, at PhilHealth number. Ipinapadala lamang ito pagkatapos ninyong lagyan ng tsek ang kahon ng pahintulot na nakasaad sa form, alinsunod sa Data Privacy Act of 2012 (R.A. 10173).

  • Ginagamit ito para sa drive na iyon lamang: para mabigyan kayo ng batch number at oras, at para makilala kayo ng aming koponan pagdating ninyo.
  • Ibinabahagi ito sa HR o sa employer na nag-organisa ng caravan, dahil sila ang namamahala ng listahan ng dadalo sa kanilang sariling lugar.
  • Hindi ito ibinebenta, hindi ipinagpapalit, at hindi ginagamit para sa ads o sa anumang uri ng tracking.
  • Walang account na nagagawa. Walang password. Walang email na hinihingi.
  • Ang PhilHealth number ninyo ay ipinapakita nang naka-mask sa ticket (halimbawa: ••••••••5678). Hindi ito ipinapakitang buo sa app.
  • Ang kopya ng ticket ninyo ay itinatago sa telepono ninyo lamang, para makita ninyo ito kahit walang signal. Puwede itong burahin sa pamamagitan ng pag-alis ng app.
  • Kung hindi kayo mag-sign up sa isang caravan, walang impormasyong umaalis sa telepono ninyo mula sa bahaging ito.

Ang GAMOT availment form

Sa bersyon 1.3, ang GAMOT availment form sa Kunin tab ay nasa loob na mismo ng app — dati itong bumubukas sa browser. Ito ang hinihingi ninyo sa amin kapag may UPSC code na kayo at gusto ninyong ihanda namin ang libreng gamot ninyo.

Kapag isinumite ninyo ang form, ipinapadala sa sariling sistema ng TrueMed ang mga sumusunod:

  • litrato ng reseta at ang pangalan ng klinika at UPSC code na nakasulat doon;
  • litrato ng valid ID at kung anong uri ito;
  • apelyido, pangalan, gitnang pangalan, kaarawan, at email kung ibibigay ninyo;
  • pirma ninyo, na iginuguhit ninyo sa screen;
  • contact number at address ng delivery (kalye, lungsod, ZIP, rehiyon, landmark at anumang tagubilin sa paghahatid);
  • kung Walk In kayo o galing sa isang kumpanya o grupo.

Ipinapadala lamang ito pagkatapos ninyong lagyan ng tsek ang kahon ng pahintulot na nakasulat sa form, alinsunod sa Data Privacy Act of 2012 (R.A. 10173).

  • Ginagamit ito para ihanda ang availment ninyo: para masuri ng parmasyutiko ang reseta, para mabuo ang mga papeles ng PhilHealth benefit ninyo, at para maihatid o maipapickup ang gamot.
  • Naka-imbak ito sa sariling Google Drive at talaan ng TrueMed. Hindi ito ibinebenta, hindi ipinagpapalit, at hindi ginagamit para sa ads o sa anumang uri ng tracking.
  • Walang account na nagagawa. Walang password.
  • Kapag kumuha kayo ng litrato ng reseta, ipinapadala ito nang isang beses sa server namin para mabasa ang klinika at ang UPSC code at maisulat na sa form para sa inyo. Mababago pa rin ninyo ang bawat sagot.
  • Wala kaming itinatago sa telepono ninyo mula sa form na ito maliban sa reference code (halimbawa: TM-20260907-N87AB) at ang petsa, para makita ninyo kung kailan kayo huling nagsumite. Walang litrato, walang pirma, at walang sagot na naiiwan sa telepono.
  • Hindi ninyo kailangang pumayag sa camera. May maliit na link sa ilalim ng pindutan na magbubukas ng parehong form sa browser.

Who we are

TrueMed Pharmacy is operated by MLT Unlimited Solutions Corp, an FDA- and DOH-licensed pharmacy and accredited PhilHealth GAMOT facility in Makati, Metro Manila, Philippines. TrueMed is not PhilHealth; eligibility and coverage are determined by PhilHealth.

Changes

If this policy changes, the updated version will be posted at this address with a new effective date.
